Social Services

Our Social Services Department provides a variety of services to hospital inpatients, outpatients, family members and the community.  They include:

• Assistance with personal, social and environmental difficulties which may interfere with obtaining the maximum benefit from medical care,
• Assistance in obtaining home medical equipment or supplies as the patient's physician prescribes,
• Coordination of home health care such as nursing or Hospice visits,
• Coordination of post-discharge care such as placement in extended-care or rehabilitation facilities,
• Referrals to community resources that may help a patient to maintain his/her independence, such as meal or homemaker services, financial assistance and available transportation services,
• Counseling to help patients and family members cope with the emotional stress of illness and to make decisions about continued care.
